How to use it
Print two copies and get a signature
A bill of lading is the contract and inspection record for the physical move. If the driver shows up without one, use ours — an unsigned or unmarked BOL makes a damage claim very hard to win.
1. Print two copies
One copy stays with you, one stays with the driver. Fill in the vehicle and contact details before the truck arrives.
2. Inspect and sign at pickup
Walk the vehicle with the driver, write down every existing scratch, dent and chip, then have the driver sign your copy before the car is loaded.
3. Repeat at delivery
Inspect again before the driver leaves, note any new damage on the BOL, and have the driver sign the delivery block.

Photograph the truck and trailer, the trailer plate, the driver's CDL, and your vehicle from the front, back, both sides and the top, and take a video of the car being loaded. Photos taken before pickup day are not accepted by insurance companies. Remove toll transponders and parking passes — tolls incurred in transit are not the responsibility of Enterprise Auto Transport. Personal items left in the vehicle are never covered by cargo insurance. All claims are filed with the carrier, which holds the cargo insurance — not with Enterprise Auto Transport.

Photograph everything the day of pickup
- Photos of the truck and trailer that is picking up your vehicle
- A clear photo of the truck / trailer license plate
- A photo of the driver's CDL
- Photos of your car before loading — front, back, both sides and the top
- A video showing your car actually being loaded onto the truck
Photos taken beforehand are not accepted by insurance companies.
Insurers want time-stamped photos and video from the day of pickup, with the truck in frame. A folder of pictures from last month will not support a claim. Do the same walk-around at delivery before the driver leaves.
All claims must be filed with the carrier, not with Enterprise Auto Transport. The assigned carrier holds the cargo insurance ($250,000 cargo coverage on open carriers; $1,000,000 cargo coverage on enclosed carriers). As a licensed broker we arrange the transport and do not carry cargo coverage on your vehicle.
What is on the form
Everything a carrier BOL should capture
- Order reference, pickup date and estimated delivery date
- Shipper and receiver names, addresses and phone numbers
- Carrier company, driver name, MC/DOT number and truck plate
- Year, make, model, color, VIN, odometer and running condition
- Condition-at-pickup damage notes with a standard damage key
- Pickup and delivery signature blocks for you and the driver
- A new-damage-at-delivery block to complete before the driver leaves
